Pet vaccination records and vet visit checklist

Use this checklist to prepare for a vaccination discussion with your veterinarian. AAHA and WSAVA guidance was checked on October 4, 2026.

A reviewed translation is not yet available for this language. This guide is displayed in English.

This page does not decide vaccine dates, doses, which vaccines are needed, whether vaccination is complete, or whether rabies law is met. A veterinarian must review age, prior records, health, exposure, vaccine products and local law together.

Bring these records

Previous vaccinations

Collect dates, vaccine and product names, clinic records and certificates. Say if any record is missing or uncertain.

Your pet’s health

Note species, birth date or estimated age, current conditions and medicines, and any reaction after a previous vaccination.

Daily life and travel

Tell the veterinarian about contact with other animals, indoor or outdoor life, boarding, grooming and travel plans.

Ask your veterinarian

  • Which vaccines should be considered after reviewing the record and current health?
  • When should the next visit or vaccine review be? What if the prior record is uncertain?
  • What rabies requirements and certificates apply where we live or travel?
  • What reactions should I watch for, and whom should I contact if one occurs?

Veterinary guidelines

These are general guidelines. Agree on an individual plan with your veterinarian.

What if I cannot find previous records?
Tell your veterinarian what is missing and share any clinic or timing you remember. This page does not guess the number of doses.
